New Patient Registration

• fill out an online registration form
• visit reception from 8.30am-5.30pm and collect a paper registration form
• we request that new patients also complete our New Patient Questionnaire which allows us to record your past medical history and care requirements whilst we away your full medical records from your previous practice - this questionnaire can be handed into the practice or emailed to: SRMP@aapct.scot.nhs.uk 

The practice provides a service to approximately 10,400 patients living in Prestwick, Monkton and Ayr, as well as some areas of Troon, Symington and Mossblown.  We welcome new patients to our practice.  It is very easy to register, by clicking on the registration form via the link above. To check if you live within our practice boundary, please ask our Reception team.

As a parent, family member or carer, you may be able to access services for someone else. We call this having proxy access. We can set this up for you with the valid consent.
