Fit Notes

If you’ve been off sick for more than 7 days

Your employer may require a fit note to support statutory sick pay.  To request a fit note:

  • telephone the practice

 

If you’ve been off sick for 7 days or less

If you’re off work sick for 7 days or less, your employer should not ask for medical evidence that you’ve been ill. Instead, they can ask you to confirm that you’ve been ill. You can do this by filling in a form yourself when you return to work. We call this self-certification.

Hospital Fit Notes

Please note if you require a fit note following discharge from hospital, your hospital consultant is responsible for providing this.

Working Health Services Scotland

Working Health Services Scotland is a free, confidential service designed to help people who are:

  • Employees of small businesses (less than 250 employees) and still attending work or absent from work (for any period of time). 
  • Self employed and still attending work or absent from work (for any period of time).

They can provide rapid access to assessment, advice and where appropriate, interventions including occupational therapy, physiotherapy and counselling.
