Clinics

Chronic Disease Monitoring

We encourage patients living with a long term chronic condition to make an annual appointment for relevant blood monitoring around their birthday.  Reviews for respiratory conditions will be carried out via telephone by our Practice Nurse.

Asthma Monitoring

The practice uses software for the monitoring of asthma.  This has been developed by a company called Inhealthcare and approved by the Health Board.  It involves a questionnaire being sent via text message for patients to complete.  The results will be reviewed by a clinician who will contact patients if necessary.  If you suffer from asthma and receive an invitation to complete a questionnaire, we would be grateful for your participation.
