Changing Your Contact Details

To update details like your name, address, phone numbers or email address:

• fill out a change of contact details form
• telephone or visit the practice.

If you have moved to a new address, please let us know.  You will need to be in our catchment area in order to remain registered with our practice.

Please provide proof if you have changed your name. This could be a marriage or civil partnership certificate, deed poll or birth certificate.
